The MI Choice Waiver Program may be able to help.
Also known as Medicaid Waiver, this program is funded by the Michigan Department of Health and Human Services and is provided by the Region 2 Area Agency on Aging.
The MI Choice Waiver Program is aimed at assisting adults so they can remain living comfortably at home with the help they need. The program focuses on individual choice and upholds that people are experts in their own lives. Participants in the program get to select the type of care they receive, who provides the care and where they receive the care. The MI Choice Waiver Program is available to those who qualify for Medicaid, who are 65 and older, or who are 18 and older with a disability and need services like those provided in a nursing facility.
Services available through this program may include community living supports (housekeeping and personal care), respite care, home modifications, chore services and assistance with connecting to other community resources.
When you enroll in the MI Choice Waiver Program, a team will assess your eligibility and need for services. They will discuss options with you and assist you in planning for your care. Once services are in place, the team will contact you by phone calls and visit you in person periodically to ensure services are meeting your needs.
